CellaVision Global Test 2025:3 – Myelodysplastic/Myeloproliferative Neoplasm with Ring Sideroblasts and Thrombocytosis (MDS/MPN-RS-T)

Case Description

A 74-year-old man presented with a two-month history of progressive fatigue, dizziness, and shortness of breath on exertion. He also reported easy bruising but denied any active bleeding, fever, night sweats, weight loss, or recurrent infections. On physical examination, he appeared pale with conjunctival pallor. No lymphadenopathy or hepatosplenomegaly was detected, and cardiovascular and respiratory examinations were unremarkable.

Case conclusion

Hematologic evaluation demonstrated anemia with reduced red blood cell count and hemoglobin concentration, accompanied by elevated white blood cell and platelet counts. Peripheral blood smear examination showed thrombocytosis with frequent giant platelets and circulating micromegakaryocytes. The leukocyte population demonstrated the presence of blasts and immature granulocytes, with hypersegmented neutrophils observed. Red cell morphology revealed hypochromia, microcytosis, poikilocytosis, and the presence of nucleated RBCs.
These findings indicate dysplastic changes involving erythroid, myeloid, and megakaryocytic lineages, suggesting an overlap syndrome.

Diagnosis: Myelodysplastic/Myeloproliferative Neoplasm with Ring Sideroblasts and Thrombocytosis (MDS/MPN-RS-T).
