CellaVision Global Test 2026:3 – Primary Myelofibrosis (PMF)
Case Description
64-year-old man with a 4-week history of increasing fatigue, weakness, night sweats and unintentional weight loss. He reports intermittent low-grade fever. No previous history of hematologic disease.
On examination he appears pale and fatigued, with a swollen abdomen.
Laboratory testing showed a slight leukocytosis (WBC 12.6 × 10⁹/L), thrombocytopenia (PLT 58 × 10⁹/L) and slightly anemic (HGB 100 g/L). The automated differential showed an increased basophil count, and the analyzer flags indicated “Blasts?”, “IG present?”, and “Atypical lymph?”.

RBC Overview:
Smear Interpretation
Peripheral blood smear examination reveals increased myelopoiesis’s with immature granulocytes (12%) and increased basophil count present.
Additional findings include Nucleated RBCs in different maturation steps as well as schistocytes, teardrop cells, and elliptocytes.
Diagnosis
Primary Myelofibrosis (PMF) is a rare blood cancer belonging to a group of disorders called Myeloproliferative Neoplasms (MPNs). In its overt fibrotic stage, PMF is highly characterized by a leukoerythroblastic blood picture, featuring an increase of immature granulocytes and nucleated red blood cells in the peripheral blood smear, as well as a high number of teardrop cells (dacrocytes).
