Case Description
A 77-year-old man with night sweats, fever, unexplained weight loss, fatigue, pruritis, lymphadenopathy, and splenomegaly.

RBC Overview
Case Conclusion
CBC and Differential: Leukocytosis, anemia, and thrombocytopenia. Increased absolute lymphocyte count due to a population of abnormal lymphocytes, most with mature chromatin, irregularly shaped nuclei, and high N:C ratios suggestive of a mature lymphoid neoplasm. Additional diagnostic tests identified mantle cell lymphoma; neutrophilic left shift, Pelger-Huet neutrophils, NRBCs at various stages of maturation and NRBCs with dysplastic features. Numerous smudge cells possibly originating from abnormal lymphocytes.
RBC morphology: anisocytosis, polychromasia, numerous echinocytes, and RBCs containing distinct pappenheimer bodies.
